We use the information you provide for purposes connected with your care and the operation of the practice, including:
- assessing, diagnosing, treating and managing your health care;
- administrative purposes involved in running the medical practice;
- billing purposes, including relevant Medicare and health insurance requirements;
- communicating with doctors, specialists and other health practitioners involved in your care, as advised by you; and
- participation in research, quality assurance and audit programmes maintained by regulatory and credentialing organisations, including specialist colleges, professional societies, universities and government agencies.
Storage and retention of your medical record
Your medical record is stored electronically on secure local servers. The supplied practice privacy statement states that records may be kept for up to seven (7) years following your last consultation with the practice.
Sharing information as part of your care
Where necessary for continuity of care, information may be shared with other health practitioners involved in your treatment. Your case may also need to be presented to a multidisciplinary team of specialists and allied health practitioners when this is required to plan or coordinate treatment.
